Abstract Submission deadline has been extended to 15th August 2008. Acceptance will be notified by the beginning of September.

Abstracts should demonstrate an initiative that has contributed to the support of patient centred care. This may include such things as the development of an educational programme or educational material, the description of a piece of research, provision of a care component, demonstration of collaborative working or a health promotion initiative. This list is by no means exhaustive.

The submissions will be grouped under 5 main categories.
1. Supporting children and young people.
2. Supporting self-management of Type 1 diabetes.
3. Supporting self-management of Type 2 diabetes.
4. Supporting prevention and early diagnosis.
5. Supporting hard to reach or disadvantaged groups.

There can be cross-over within the groups, as an initiative to address obesity in childhood could appear in categories 1 or 4, or describing an educational programme for minority ethnic groups could come under 3 or 5.

Abstracts should be no more than 250 words. Poster size should be A0.
